Terms & Conditions

Registration with Pharmatemp Locum Services Pty Ltd (ACN 694 887 664)
is accepting and agreeing to the following Terms and Conditions:

Provision of Locum Pharmacists

Pharmatemp Locum Services Pty Ltd (the Supplier) provides qualified and registered Pharmacists to the Client upon request.

 

Client Responsibilities

The Client is required to submit all relevant information on their Registration Form and must notify Pharmatemp of any changes as they occur.

The Client must also ensure that the workplace provided for the Relieving Pharmacist complies with all current safe workplace and Occupational Health & Safety (OH&S) standards.

 

Privacy Compliance

Both the Supplier and the Client agree to uphold and comply with the information privacy principles outlined in any applicable Privacy Act of the Commonwealth of Australia or the State of Victoria. This includes the secure use and disclosure of personal information related to the Relieving Pharmacist.

 

Relieving Pharmacist Obligations

The Relieving Pharmacist is required to truthfully provide all relevant information regarding their qualifications and experience on their Registration Form and must inform the Supplier of any changes.

The Relieving Pharmacist must hold a current PHA Registration and current Public Liability Insurance.

Payment Terms and Charges

The Supplier and the Client will agree on the hourly rate as well as any additional charges such as travel allowance, accommodation, and superannuation for the supplied Relieving Pharmacist.

The Relieving Pharmacist will invoice the Client directly, and the Client is obligated to pay the Relieving Pharmacist’s invoices promptly.

The Supplier charges a daily placement fee for their services to the Client . 

The Supplier will invoice the Client at the end of each calendar month for all bookings made. The Client agrees to settle the invoice within 14 days of receipt.

The Supplier reserves the right to apply a Late Fee to overdue invoices at their discretion.

 

Permanent Employee Placement Services

The Supplier may assist the Client in finding Permanent Employees.

All details and the Permanent Placement Fee will be tailored to the Client’s needs and must be agreed upon before any suitable candidates are referred to the Client.

 

Arrangement of Work Placements

Both the Client and the Relieving Pharmacist agree that all work placements must be arranged through the Supplier and not directly between themselves. Any direct arrangements between the Client and the Relieving Pharmacist must be reported to the Supplier.

Restrictions on Future Employment

The Client agrees not to offer the Locum Pharmacist any full-time, part-time, or temporary employment at any pharmacies owned by them for a period of six months following any employment arranged by the Supplier.
